1. Good Cooperation - nothing loose or broken, wearing elastics as directed, etc
2. Being on time - signing in and brushing your teeth before your scheduled appointment time
3. Wearing your Hosier orthodontics T-shirt to your appointment
4. Good Brushing - earning a grade of A or B
5. Dental exam - proof of cleaning and exam appointment with your dentist
